Introduction. Endometritis and elder fertile age are the one of the main reason of infertility. Molecular methods of endometrial investigation have opened new perspectives in diagnostics and evaluation of the efficacy of the management of endometritis Objective – the comparative assessment of the endometrial immune status and apoptosis level in the young and elder reproductive age in female healthy persons and patients with endometritis Methods. In the work there was used a method of creation of a primary culture of endometrial cells, immunocytochemical method, light microscopy, morphometry and statistical data processing. Results. Normal age-related decreasing of endometrial apoptosis involves only caspase-dependent apoptosis (FasL, р53, Caspase-3), whereas mitochondrial apoptosis is unchanged. At that, in young and elder aged females endometritis was accompanied by the decline in both caspasedependent and mitochondrial apoptosis by 2,7–3,4 times. In normal endometrium of females of various aged groups there was observed some increase in T-helpers number, while the number of other subsets of immune cells did not altered. In females of elder reproductive age with endomentritis the number of NK-cells, cytotocsic T-cells, B-cells and T-helpers elevated by 2–5 times. It indicates to the pronounced immunopathological response in females of elder fertile groups with endometritis. Conclusion. Chronic endometritis in females of elder fertile aged groups is characterized by more pronounced disorder of the immune status and apoptosis than in young females. We can recommend the investigation of the expression of CD8, CD16, AIF in endometrial cells for the diagnostics of endometritis.
