Abstract: This review summarizes the results of studies devoted to identification the dominant mechanisms of formation and persistence of inflammation in severe asthma. According to the latest research, the most likely causal factors and molecules that underlie the formation of severe asthma and therapy resistance can be divided into the following groups: the imbalance of cytokine profile, the resistance to glucocorticoids, angiogenesis and remodeling of the bronchi, the shift of the immune response towards the Th2. These mechanisms could potentially be considered for diagnostic purposes, and become the new targets of asthma therapy.
