Гепатоцеллюлярная карцинома (ГК) – наиболее часто встречающаяся форма злокачественных опухолей печени, характеризующаяся поздними сроками выявления и высокой агрессивностью течения. Основными факторами риска развития ГК являются хронические инфекции вирусами гепатита В и С, цирроз, токсическое отравление печени и ряд наследственных метаболических заболеваний. Гепатоканцерогенез представляет собой многостадийный процесс, сопровождающийся множественными изменениями структуры ДНК и экспрессии генов на разных этапах прогрессии опухолей. Эти нарушения затрагивают прежде всего компоненты Wnt/β-катенин-, р53- и Ras/MAPK-зависимых сигнальных путей, системы контроля укладки хроматина и ответа на окислительный стресс. Однако до настоящего времени не предложено достаточно надежных прогностических маркеров для оценки уровня злокачественности ГК, которые бы способствовали выбору оптимальных индивидуализированных схем лечения этого заболевания. Развитие технологий массового параллельного секвенирования (NGS) открывает значительные перспективы в этом направлении. Методы экзомного и транскриптомного секвенирования позволили выявить новые однонуклеотидные замены, инсерции и делеции генов, крупные хромосомные перестройки, слитные белки, характерные для опухолевой ткани, дифференциальную экспрессию генов и их альтернативно сплайсируемых изоформ. В обзоре рассмотрены основные молекулярно-генетические нарушения, характерные для гепатоканцерогенеза, и возможности использования NGS- технологий для идентификации ключевых детерминант злокачественной трансформации гепатоцитов, которые могут рассматриваться как перспективные мишени для разработки новых подходов к терапии ГК.
