Туберкулез является 2-й по значимости причиной смерти в мире от инфекционного агента; это заболевание представляет собой важную социальную и медико-биологическую проблему. Препарат 1-й линии изониазид – один из самых эффективных противотуберкулезных препаратов, однако он обладает широким спектром побочных реакций, среди которых – индуцированное поражение печени, способное привести к смерти пациента. Этот спектр побочных реакций объясняется наличием высокотоксичных промежуточных соединений; риск их возникновения может быть частично определен при генотипировании пациента по полиморфизмам генов, участвующих в метаболизме препарата, и по выведению продуктов его метаболизма из организма. В нескольких метаанализах подтверждена взаимосвязь между фенотипом ацетилирования по гену N-ацетилтрансферазы-2 и риском возникновения тяжелых побочных реакций или ранней неудачи лечения. Также в литературе предложена и опробована схема коррекции лечения в соответствии с профилем ацетилирования пациента (быстрый или медленный ацетилятор), значительно снижающая риски нежелательных исходов. Существуют данные о взаимосвязи полиморфизмов генов и гиперметилирования CpG-участков цитохромов P450 CYP2E1 и CYP2D6, а также нулевым типом гена глутатион-S-трансферазы GSTM1 с повышенным риском индуцированных поражений печени. Есть и другие генетические вариации, вносящие вклад в токсичность противотуберкулезных препаратов, требующие более подробных исследований.
