ГЕНЕТИЧЕСКИЕ АСПЕКТЫ НЕКЛАССИЧЕСКОЙ ФОРМЫ ВРОЖДЕННОЙ ГИПЕРПЛАЗИИ КОРЫ НАДПОЧЕЧНИКОВ: ЛИТЕРАТУРНЫЙ ОБЗОР И СОБСТВЕННЫЙ ОПЫТ

Врожденная гиперплазия коры надпочечников представляет собой группу заболеваний, возникающих вследствие дефектов генов, кодирующих ферменты стероидогенеза. Причиной ее развития чаще всего служит дефект гена CYP21A2, приводящий к нарушению синтеза 21-гидроксилазы. В зависимости от выраженности клинических проявлений, соответствующей степе- ни снижения активности фермента выделяют классическую и неклассическую формы заболевания. Основным методом диа- гностики в настоящее время является определение в сыворотке уровня 17-гидроксипрогестерона, субстрата катализируемой 21-гидроксилазой реакции, что позволяет выявить классическую форму заболевания. Для диагностики неклассической формы, часто проявляющейся незначительным повышением уровня 17-гидроксипрогестерона и отсутствием яркой клинической карти- ны, оптимальным методом является генотипирование. Понимание генетических особенностей позволяет выявить заболевание, прогнозировать тяжесть его течения, определить риск развития заболевания у потомства. В статье подробно рассматрива- ются структура гена CYP21A2, закономерности появления и различные варианты мутаций, механизм развития клинических проявлений. Кроме этого, разработанный генетический тест позволяет определить 15 основных мутаций при врожденной ги- перплазии надпочечников. Отдельное внимание уделено роли гетерозиготного носительства, в том числе приведены результа- ты, полученные в ходе молекулярно-генетического исследования 85 пациентов с симптомами гиперандрогенемии.
