ЛУЧЕВЫЕ И МОРФОЛОГИЧЕСКИЕ СОПОСТАВЛЕНИЯ ПРИ ДИАГНОСТИКЕ ГАМАРТОМЫ СЕЛЕЗЕНКИ

Цель. Анализ собственных данных по диагностике, морфологической верификации и лечению гамартомы селезенки и сопоставление их с данными литературы. Материал и методы. В Институте хирургии им. А.В. Вишневского накоплен опыт диагностики и лечения более 300 пациентов с различными очаговыми образованиями селезенки, но с гамартомами было лишь 4 больных (2 женщины и 2 мужчин в возрасте 50–58 лет). Пациентам проводили обследование, включавшее клинические данные, УЗИ с дуплексным сканированием, мультиспиральная компьютерная томография (МСКТ) и магнитно-резонансная томография (МРТ). Все пациенты были прооперированы, образования морфологически верифицированы (гистологическое и иммуногистохимическое – ИГХ-исследование). Результаты. Описаны сложности диагностики и дифференциальной диагностики гамартомы селезенки по данным УЗИ, МСКТ, МРТ. В 3 случаях были заподозрены злокачественные образования селезенки, что в конечном итоге определило объем и характер выполненных хирургических вмешательств. Селезенка была сохранена в 2 случаях. Отдаленные результаты прослежены у всех больных. Признаков рецидива опухоли не выявлено ни в одном наблюдении. Лучшие отдаленные результаты, включая качество жизни, отмечены у пациентов после резекции селезенки. Заключение. Приведенные наблюдения представляют интерес в связи с редкой встречаемостью гамартомы селезенки, что во многом объясняет возникшие трудности дооперационной диагностики. При наличии гиперваскулярного образования в селезенке в дифференциальный диагностический ряд следует включать гамартому. В неясных диагностических случаях срочное интраоперационное морфологическое исследование помогает поставить правильный диагноз, что, в свою очередь, дает возможность выполнить органосберегающее оперативное вмешательство (если есть техническая возможность) и обеспечить более высокое качество жизни в послеоперационном периоде. ИГХ-исследование является важным дополнительным методом дифференциальной диагностики гамартомы селезенки.
