Сахарный диабет (СД) является опасным вызовом всему мировому сообществу и приоритетом первого порядка национальных систем здравоохранения всех без исключения стран мира. Растущий уровень гипергликемии при СД типа 2 (СД2) ассоциируется с нарастающей инсулинорезистентностью и относительно или абсолютно нарушенной чувствительностью к инсулину. Большинство лекарственных средств, применяющихся при терапии СД2, имеют инсулинозависимый механизм действия, целью которого является либо стимуляция секреции инсулина, либо улучшение метаболизма глюкозы. Недавно появился принципиально новый инсулиннезависимый подход к лечению СД2 – снижение уровня гликемии путем ингибирования белков котранспортеров SGLT2 (sodium glucose transporter) в проксимальных канальцах почек, которые отвечают за обратную реабсорбцию глюкозы. Препараты группы ингибиторов SGLT2-котранспортеров эффективны как в режиме монотерапии, так и в комбинации с другими сахароснижающими препаратами. По результатам проведенных рандомизированных контролируемых исследований, терапию данным классом препаратов пациенты переносили хорошо. Частота случаев гипогликемии статистически не отличалась от таковой в группе плацебо. На фоне терапии наблюдалось стабильное снижение массы тела у пациентов с СД2. Терапия всеми препаратами класса ингибиторов SGLT2-котранспортеров сопровождалась снижением АД и массы тела. Препараты данного класса могут играть важную роль на ранних этапах развития диабетической нефропатии, оказывая потенциальное нефропротективное действие, работая на уровне снижения внутриклубочкового давления одного нефрона. Инсулиннезависимый механизм действия и низкий риск гипогликемии делают ингибиторы SGLT2-котранспортеров эффективным и безопасным дополнением к существующим методам контроля гликемии у больных СД2.
